Medical Nutrition Therapy for Chronic Disease Management

Our dietitians specialize in medical nutrition therapy (MNT), a proven approach to managing chronic conditions through individualized diet and lifestyle interventions.     Our MNT services can help manage:

    Diabetes Education

    Diabetes Self-Management Education/Training (DSME/T) is the ongoing process of facilitating the knowledge and skills needed to manage diabetes. DSME/T is appropriate for all individuals with all types of diabetes, including women with gestational or pre-existing diabetes in pregnancy.

      The American Diabetes Association recommends all patients be referred for diabetes education:

      1. With a new diagnosis of diabetes
      2. Annually for health maintenance
      3. When new complicating factors influence self-management
      4. When transitions in care occur

      All consultations are one-one-one with a diabetes educator.

      Consultations Include:

      • Gaining a better understanding of diabetes and how it’s treated

      • Instruction on blood glucose monitoring

      • Review of blood sugar levels in search of patterns and modifications to improve management

      • Instruction on continuous glucose monitoring and review of data

      • How to use new medications, administer insulin, or use insulin pumps

      • Learning to prevent and treat acute complications and manage long-term complications

      • Gaining understanding of gestational diabetes, including causes, management, and ways to lower risk for diabetes post-partum

      • How to manage your blood sugars by learning to understand carbohydrates, fats, and proteins, and how to balance them throughout the day

      • Understanding healthy eating, carbohydrate counting, label reading, and insulin to carb ratios

      How to Get Started with Diabetes Education

      Contact your primary care provider today for referral to the diabetes education program.  Referrals can be faxed to 773.594.8454.

      Although Medicare and most insurance plans cover diabetes education, please contact your insurance provider to determine coverage of services.

        Are nutrition and diabetes services covered by insurance?

        Yes, many insurance plans—including Medicare and Medicaid—cover nutrition services for conditions like diabetes and kidney disease, however it is always best to call your insurance to verify services covered under your policy. We’ll verify your coverage before your first visit.

        Do you offer telehealth?

        Yes, however coverage is dependent on your insurance policy. We strongly advise calling your insurance to verify your coverage.

        What should I expect during my first appointment?

        Your initial visit will include a full nutrition assessment, a review of your health history and eating habits, and the creation of a personalized plan based on your goals.

        Can I get a custom meal plan?

        Yes. Our dietitians work with you to create realistic meal plans tailored to your health conditions, dietary restrictions, preferences, and lifestyle.
